Ce dispositif, accessible en ligne, propose aux professionnels de sant\u00e9 d\u2019endosser le r\u00f4le d\u2019un soignant confront\u00e9 aux sp\u00e9cificit\u00e9s du parcours de patients atteints de cancer.<\/p>\n
Le sc\u00e9nario porte sur plusieurs \u00e9tapes cl\u00e9s de la conciliation m\u00e9dicamenteuse : recueil structur\u00e9 des traitements habituels, des traitements oncologiques et des co-prescriptions ; identification des risques d\u2019interactions m\u00e9dicamenteuses ; conduite d\u2019un entretien approfondi avec le patient (prise en compte de l\u2019autom\u00e9dication, des compl\u00e9ments alimentaires, de la phytoth\u00e9rapie, des traitements de support et de la gestion des effets ind\u00e9sirables) ; communication et coordination avec l\u2019\u00e9quipe pluriprofessionnelle. <\/p>\n
L\u2019objectif affich\u00e9 est de renforcer les comp\u00e9tences des professionnels en mati\u00e8re d\u2019entretien pharmaceutique et de contribuer \u00e0 la s\u00e9curisation des prises en charge m\u00e9dicamenteuses en oncologie.
